Traumatic Brain Injury Medicaid Waiver Program

The Traumatic Brain Injury (TBI) Medicaid Waiver Program offers a vital pathway for New Yorkers with traumatic brain injuries to live independently in the most appropriate, least restrictive settings. Administered by the Southern Tier Independence Center (STIC) as the Regional Resource Development Center (RRDC) for the Southern Tier, this program supports eligible individuals in transitioning from nursing homes to community living with essential services or helps those at risk in the community avoid institutionalization through tailored supports like personal care, home modifications, and assistive technology.

As the single point of access for TBI Waiver enrollment in the region, STIC’s Regional Resource Development Specialists provide personalized guidance, advocacy, and service coordination to ensure participants receive comprehensive, person-centered care plans that promote independence and quality of life. Under contract with the New York State Department of Health, STIC’s dedicated team empowers individuals to thrive in their preferred community settings.


Eligibility - Active community based long term care Medicaid coverage, between the ages of 18-64 upon application, documentation of brain injury resulting in disability, requires a nursing facility level of care, choose to participate in the program, available services are adequate to meet the individual’s needs in a community setting.
