Community habilitation
Community Habilitation is a Medicaid funded program operated under OPWDD to provide supports to people with developmental disabilities to develop or enhance the skills needed to live more independently in their homes or their community. Being a part of a community is an essential part of living a full life. STIC is dedicated to building meaningful connections between people and their community. Community habilitation gives people who have disabilities the skills they need to become a more active part of their community, build relationships with others and to simply, have fun.
Our person-centered approach to community habilitation helps people with developmental disabilities to improve their independent living skills and participate in social, recreational activities. Every person gets community habilitation support tailored to meet their unique needs.
Eligibility
A person must meet eligibility criteria as defined by the New York State Office for People with Developmental Disabilities (OPWDD). A referral should be made by your Care Manager.

The Front Door staff will guide you through the steps involved in finding out if you are eligible for services with OPWDD, identify your needs, goals and preferences, and help you work on a plan for getting those services, including Community Habilitation.

Become a Direct Support Professional!
As a Direct Support Professional, your role as an essential worker includes supporting individuals to learn skills to become more independent.
- Flexible hours as they are based on the needs of the individual.
- Valid driver’s license/personal vehicle required
- Extensive travel throughout four counties; Broome, Tioga, & Chenango.
