MONEY FOLLOWS THE PERSON

Money Follows the Person (MFP) Open Doors is funded through the NYS Department of Health Money Follows the Person Project.  Locally, STIC works with AIM Independent Living Center of Corning, Access to Independence of Cortland and Catskill Center for Independence in Oneonta to provide services in 11 counties.

Open Doors assists individuals in nursing facilities, OPWDD Intermediate Care Facilities (ICF), and hospitals to return to living independently in the community or to transition to smaller OPWDD IRAs (group homes 4 beds or less) with home-and community-based services.

Open Doors Transition Specialists complete intakes with program participants, advocate on their behalf, and give support as needed.  Transition Specialists coordinate with participants, family and friends, Medicaid, Social Workers, Residence Managers, and community service agencies to create a safe discharge plan. Transition Specialists also keep in regular contact with participants who qualify for the first 12 months after their transition by providing ongoing support and assisting with the skill-building necessary for continued independent living.

Throughout the transition process, participants also have the opportunity to have regular one-on-one contact with an MFP Peer, who provides a listening ear, encouragement, and advocacy.  After their transition, participants are offered Person Centered Coaching for 6-8 skill building sessions to help identify strengths, preferences, needs, and desired outcomes.  

MFP Open Doors also works closely with the Olmstead Housing Subsidy (OHS) Program, which provides housing assistance, furniture, and household goods to those who qualify.

Eligibility

Be at least 18 years of age and a current resident in a nursing facility, ICF (5 or more beds), or a hospital.

Referrals may be made by anyone via phone, fax, or email.
